Paperwork Due July 25th
Equipment will not be issued until you have completed ALL of the paperwork requirements.
Equipment will be issued:
- July 26 for C/O 2018
- July 27 for C/O 2019
- July 28 for C/O 2020

You must complete the following items prior to August 1 so you do not miss the first day of practice.
1. Virginia High School League Pre-participation Exam (PPE) Form
Students must have a completed physical exam prior to participating in high school sports. The exam must be completed by a MD, DO, NP or PA on the current VHSL form. The form must be completed by the physician after May 1, 2017 to be valid for the 2017-18 school year. Completed forms should be turned in to the Activities Office. Do not give it to a football coach!
The VHSL has released a new version of the PPE form dated February 2017. While this form of the version is preferred, schools may accept both the current version and the previous version (dated April 2016) for the 2017-18 school year. Please aware of the signatures that are required on the following pages:
- Bottom of page 1 (student)
- Bottom of page 2 (parent and student)
- Bottom of page 3 (Doctor’s signature, address and date)
- Bottom of page 4 (parent signature - two locations)
Tips:
- FCPS Athletic Training Programs are offering Pre-Participation Screenings at select area high schools until June 22 for $50.
- If you have Kaiser insurance, be aware that they are notorious for delaying paperwork for several weeks so get it done early!
- Your athlete must have health insurance in order to participate. If you do not have insurance, FCPS offers voluntary student insurance. Be sure to purchase a football specific plan.

2. Concussion Education for Parents & Athletes
Parents must complete the concussion education program prior to the student’s participation in high school sports. Parents need only complete the program one time for all their children participating in FCPS athletics if they register each child's FCPS ID number prior to viewing the slideshow. 3. Emergency Care Card
You will need to complete a new emergency card card for each season that your athlete participates on a team. Please turn in two copies to the Activities Office. The athletic trainers will maintain one copy and the coaching staff will maintain the other.

4. FCPS Participation Policy
Document stating that student activities is a privilege and not a right and that you and your athlete agree to abide by codes of conduct and rules set forth by coaches, teachers, VHSL, and FCPS Regulation 2601 Students Rights and Responsibilities (SR&R).
The 2017-18 form has not yet been released.

Summer Vacations
We get it, family vacations and visiting grandma are awesome! We ask that you use the month of July to get all of your traveling out of the way. We want our athletes to be fully focused on football come the first day of practice on August 1 for varsity and junior varsity players. We only have 20 practices before the first game of the season and every one of them is essential. Every day we are implementing new schemes and techniques. Missing even one practice during this critical pre-season phase impacts your athletes and the teams development.
